Available to Dr. Neeley's hygiene patients.

To Qualify for Lifetime Whitening

  • Patient must maintain routine hygiene appointments on schedule. (meaning a minimum of 2 in a year at 6 month intervals)
  • Hygiene Appointments maybe spaced 3, 4 and 6 months apart.
  • If you alternate your cleaning with the Periodontal Dentist, we honor shared patients.
  • A Whitening gel syringe is the gift for the patient on the Lifetime Whitening Program with their routine hygiene scheduled appointment.
  • The Whitening Gift will be given at the end of the appointment with the Toothbrush Gift.

The purpose of this program is to reward our patients for keeping their preventative care current. Therefore, helping our patients prevent any major dental situations to a minimal occurrence.

Notes:

  1. The Lifetime Whitening Program will continue as long as the materials are available.
  2. Substiution of the material might have to happen if the current whitening becomes unavailable.
  3. The individual Whitening for Life Program will expire if the patient fails to keep the regular routine hygiene appointment (within a month of the proposed appointment).
